APPLICANT REQUESTS THAT:
His home of record (HOR) be corrected to reflect West Haven,
Utah.

APPLICANT CONTENDS THAT:
He owns a home in Utah and plans to retire there. He would like
his home of record changed to reflect his Utah address.
The applicant submits no supporting documentation.
The applicants complete submission is at Exhibit A.

STATEMENT OF FACTS:
The applicant is a member of the Air Force serving in the grade
of captain. His DD Form 4/1, Enlistment/Reenlistment Document
Armed Forces of the United States, reflects Albuquerque, New
Mexico as his home of record at the time of his enlistment. The
New Mexico address is also reflected on the order that appointed
him as a second lieutenant into the Air Force Reserves.

AIR FORCE EVALUATION:
AFPC/DPSIPS recommends denial. The DD Form 4/1 is the source
document for the home of record and place of entry. The Joint
Federal Travel Regulation for Uniformed Service Members Appendix
A, states the place recorded as the home of the individual when
reinstated, reappointed, or reenlisted remains the same as that
recorded when commissioned, appointed, enlisted, inducted or
ordered into a tour of active duty unless there is a break in
service of more than one full day. Only if a break in service
exceeds one full day may a member change the HOR. The HOR is
the location identified by the individual upon entering the
service, not a location where the individual is later assigned
or a location selected for convenience. A correction must be
fully justified. Domicile or legal residence may change, but
the HOR will not.
The applicant initialed the DD Form 4/1 dated 25 August 2003
listing Albuquerque, New Mexico as his home of record. There is
no error or justification that warrants a change in his home of
record.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the
applicant on 6 December 2013, for review and comment within
30 days (Exhibit D). As of this date, this office has received
no response.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by
existing law or regulations.
2. The application was timely filed.
3. Insufficient relevant evidence has been presented to
demonstrate the existence of error or injustice. The
applicants contentions are duly noted; however, he has provided
no evidence which would lead us to believe his home of record or
his place of entry currently reflected in his records were
incorrectly recorded at the time he initially came on active
duty. Therefore, we agree with the opinion and recommendation
of the Air Force office of primary responsibility and adopt its
rationale as the basis for our conclusion that the applicant has
not been the victim of an error or injustice. In the absence of
evidence to the contrary, we find no basis to recommend granting
the relief sought in this application.

The applicant be notified that the evidence presented did not
demonstrate the existence of material error or injustice; that
the application was denied without a personal appearance; and
that the application will only be reconsidered upon the
submission of newly discovered relevant evidence not considered
